Community Support Advisor (Castle Point)
A Fantastic Opportunity Has Arisen to Join CAVS as a Community Support Advisor (Castle Point)
We are seeking a compassionate and proactive individual to support residents of Castle Point as they return home following a hospital admission at Southend Hospital.
Working closely with hospital staff, particularly within the discharge team and across various wards, you will assist patients in preparing for a safe and smooth transition back home. This includes identifying and discussing any immediate needs or concerns that may impact their wellbeing, with the aim of preventing unnecessary re-admissions to hospital.
The role will ideally suit someone who is caring and confident, with excellent communication and listening skills. You’ll also need to be comfortable liaising with both healthcare professionals and patients (and their families), ensuring that everyone is supported throughout the discharge process.
Strong administrative abilities are essential, along with a commitment to maintaining confidentiality and following set processes and procedures at all times.
Applicants must have their own transport and a good knowledge of the Castle Point area.
