WELLBEING LINK COORDINATOR – CANVEY ISLAND

A fantastic opportunity has arisen to join CAVS Social Prescribing Service as a full-time Wellbeing Link Coordinator, located at CAVS Office in Canvey Island.

This hugely rewarding and varied role will involve intrinsically working with CAVS Team members, local GP practices and other professionals in the Primary Care Network to assist residents in the locality with their social, emotional and practicable needs, helping to create a person-centered approach to improve their health and wellbeing.

There is significant interaction with other agencies and the linking of people to community, voluntary and other statutory services for advice and guidance.

You will be an excellent listener, resilient, empathetic, with the ability to manage a busy diary and multi-task. Perhaps you already have skills and experience of working with vulnerable adults in healthcare, social care or other applicable settings, with knowledge and experience that you could bring to the role?
