WELLBEING LINK COORDINATOR (SOCIAL PRESCRIBER) RAYLEIGH

FANTASTIC OPPORTUNITY – Wellbeing Link Coordinator (Social Prescriber) for Rayleigh

Salary: £25, 240

Hours: 37.5 per week

A great vacancy has arisen to join CAVS, a not-for-profit charity located in Benfleet, Essex as Wellbeing Link Coordinator for Rayleigh.

Working with GP practices and the CAVS team, referrals will be received from a wide range of agencies in order to provide personalised support to individuals, their families and carers to take control of their health and wellbeing; taking an holistic approach based on the person’s priorities and a focus on ‘what matters to me’.

There is significant interaction with other agencies and the linking of people to community, voluntary and other statutory services for emotional and practical support.

If you have excellent communication and organisational skills, with ideally experience of working with vulnerable adults in healthcare, social care or other applicable settings, then we would like to hear from you.
